You should provide the full credits, including all the authors that participated to the creation of this map. If you edited Valve's map, you should include them in the credits as well.

There is nothing wrong about the fact you gave yourself credit (because it is not a retexture rather than an extension) but that the fact that you did not give credit to Valve.

Lighting is horrible in your additions, should have taken more time to make it blend in with the rest of the map. Also credits are wrong and adding your name in the map is kinda cheap really, why not make some little room outside of the map that people can't get to if you really want it.
